The Middlesex London Ontario Health Team and the Middlesex London Primary Care Network, who are supported by funding from the Government of Ontario, have jointly developed promotional materials to support community partners in helping people without a primary care provider register with Health Care Connect.
As part of a regional commitment to attach all residents to a family doctor or nurse practitioner by 2029, Health Care Connect is a key enabler – providing a standardized and equitable process to identify need and support timely attachment. Strong progress is being made in clearing the current waitlist and, to continue attaching people to care, your support is needed to guide more individuals who are seeking primary care to register.
