Seamless Care Optimizing the Patient Experience (SCOPE) is a free virtual, inter-professional care team that connects primary care providers (PCPs) to a single, centralized, standardized point-of-access. It offers:
- Specialist referral navigation and support
- Expedited imaging access to ultrasound and CT scans
- Direct radiologist consults
