Seamless Care Optimizing the Patient Experience (SCOPE)

Seamless Care Optimizing the Patient Experience (SCOPE) is a virtual, inter-professional care team that connects primary care providers (PCPs) to a single, centralized, standardized point-of-access to support health system navigation, provide real-time consultation and support patients with complex and urgent needs.

SCOPE was launched in June 2024 in Middlesex London with an initial focus on diagnostic imaging (DI) departments within the hospitals. The program will allow PCPs to have real-time access to a shared virtual interdisciplinary team, providing a streamlined process to access urgent diagnostic imaging for their patients. The goal is to improve patient care and decrease unnecessary emergency room visits.

The primary hospital sites include London Health Sciences Centre – Victoria Hospital, London Health Sciences Centre – University Hospital, and St. Joseph’s Health Care London – St. Joseph’s Hospital. A secondary site will be the Middlesex Hospital Alliance – Strathroy Middlesex General Hospital (SMGH).

With the success of the initial launch, London X-Ray Associates was onboarded in March 2025 as the first Integrated Community Health Services Centre.
