The Middlesex London Ontario Health Team (MLOHT) is thrilled to be part of HealthPathways, a clinician-designed tool that supports health-care professionals in providing locally relevant, evidence-based guidelines and resources.
In partnership with the Burlington Ontario Health Team and the Greater Hamilton Health Network, HealthPathways enhances health-care outcomes across the three communities through a shared approach, marking a milestone for integrated health systems that will enable clinicians to deliver more efficient and effective care and improve outcomes.
HealthPathways allows for clinical guidance in real-time by creating a single access point among health-care providers to simplify clinical best practices, local navigation options, and patient education. It gives health-care professionals guidance for quick and accurate referrals, access to educational material to share with patients, fosters collaboration across specialties, and allows for more coordinated care and greater options in managing patients.
